Scholarship Application Form
SCHOLARSHIP
Each year, the Burley Rotary Club grants several $750 scholarship awards.
Any graduating senior in the Cassia-County School District is eligible to apply. This is an academic scholarship and awards are to be sent to the desired college or institution of higher learning of the applicant. Considerations for the scholarship will be based on a combination of the following criteria:
* Academic achievements
* Recommendations from your teachers
* School and Community Involvement
* Essay response
TO BE CONSIDERED FOR THIS SCHOLARSHIP, PLEASE COMPLETE THE FOLLOWING:
- Obtain an official High School Transcript of your grades. The transcript must be a PDF file that can be uploaded on this form.
- Ask two separate High-School teachers to provide a letter of recommendation on your behalf. These letters must be sent to the Burley Rotary Club BY THE TEACHERS. We recommend you provide your teachers with pre-addressed and stamped envelopes. All letters of recommendation must be post-marked NO LATER THAN APRIL 10th. Letters of recommendation should be sent to:

NOTE: Scholarship winners will be required to provide their student identification number for the school they will be attending to ensure the scholarship payment is properly applied.

AN EXAMPLE OF SERVICE ABOVE SELF:
Dorothy Uscola was a long-time member of the Burley Rotary Club and was our club’s first woman president (the first of many to later follow.) Her life was a wonderful example of what it means to serve others. For decades Dorothy would play the piano for Rotary-club meetings, her own church services , and countless other community events. She selflessly served as a member of many local boards and groups including: Bobcat Boosters, the Cassia Healthcare Foundation, PEO, the King Fine Arts Auxiliary, Mt. Harrison Heritage Foundation, Community Concert Board, Iris Homes and Gardens, and the Heyburn Alumni Association. Dorothy was a multi-time Paul Harris Fellow recipient having donated thousands of dollars to global causes like ending Polio and supporting education – both locally and abroad. Her love for education and the goal of helping the younger generation to become their best selves was always a joy to her. It is with her endowment (a final act of selfless service) that we are able to present these scholarships to so many of the youth in the Cassia-County School District.
